1. Our Commitment to Your Privacy

The Tikvah Center for Jewish Recovery & Healing ("the Center") is committed to protecting your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the HIPAA Omnibus Rule of 2013. We are required by law to maintain the privacy of your PHI and to provide you with this notice of our legal duties and privacy practices.

2. How We May Use and Disclose Your Information

We use and disclose your PHI for treatment, payment, and healthcare operations. Specific examples include:

  • Treatment: Coordinating your "Jewish attuned" therapy, including spiritually integrated psychotherapy and mutual support groups.
  • Healthcare Operations: Conducting quality assessments, staff training, and clinical supervision of behavioral health trainees.
  • Business Associates: We may share information with third-party "Business Associates" (such as billing services or IT providers) who perform functions on our behalf. These partners are required by written agreement to provide the same level of privacy protection as the Center.
  • As Required by Law: We will disclose PHI when required by federal, state, or local law, including reporting "sentinel events" to regulatory bodies like The Joint Commission (TJC) when applicable.

3. Safeguarding Your Data

The Center employs strict technical and physical safeguards to protect your data:

  • Electronic Security: We utilize HIPAA-compliant cloud servers and Microsoft 365® Business Premium with signed privacy agreements.
  • Storage Protocols: Client data is never stored on local computer hard drives. It is saved only to encrypted, password-protected hardware or compliant cloud platforms.
  • Communication: Staff are instructed to use encrypted email services (SecureMyEmail) when transmitting PHI.

4. Your Rights Regarding Your PHI

You have the following rights regarding the health information we maintain about you:

  • Right to Access: You may request to inspect and receive a copy of your PHI maintained in a "Designated Record Set".
  • Right to Amend: If you feel the information we have is incorrect or incomplete, you may request an amendment.
  • Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.
  • Right to Request Restrictions: You have the right to request a restriction on the PHI we use or disclose for treatment, payment, or healthcare operations.
